I am applying for DSA and need my University to complete part of the form, can you do this?

In most instances we can complete the section of a DSA application form that requires completion by your institution. Please bring your form to the Aston Webb Student Hub, C Block of Aston Webb to request this.

Please enclose a self-addressed envelope for the form to be posted back to you.

You can also email the form to disability@contacts.bham.ac.uk once you have completed all the relevant sections of the application form. Please include whether you would like us to email the form back to your or email it to Student Finance once we have complete our section.

Please note that we will need to check the form and your student record before we can confirm that we are able to complete it.
