How do I fill in the on-line form?
We advise that you only submit your application once you have consulted with your Programme Administrator, Academic Tutor/Supervisor or Wellbeing Manager to ensure you understand the process and its implications.
The leave of absence form will prompt you to check your details and provide information about your request with corresponding evidence.
